Change of account

The change applies to all persons listed on the same premium invoice. The new account must be in the name of the person paying the premium. Please note that a retroactive change is possible at the earliest to the first day of the current month.

Nota bene: If payment by LSV+ or Debit Direct is set up on the old account, this will be cancelled with the change. If you want to automatically transfer the premiums using the new account, fill in the form for that purpose.

You will find your insurance number on the back of your health insurance card or on your insurance policy. Your insurance number has at least 10 and maximum 11 digits.
